Why there is a volume limit that is not crossed

Removing large volumes means fluid losses, wide areas of tissue trauma and longer operating time — each of which raises risk. Limits are therefore placed on the volume in a single session, and staging across two operations may be the safer decision rather than attempting everything at once. A patient asking for “everything in one operation” is told why the answer is no, when no is the correct answer.

Pre-operative assessment

Assessment covers general health, medications, surgical history, thrombosis risk factors and the necessary laboratory work, alongside examination of skin and tissues. Some patients need preparation or better control of a chronic condition before a date is set — that is part of the plan, not a delay to it.

Fat transfer within the plan

Removed fat may be used to restore volume elsewhere. It is living tissue and a portion is reabsorbed, as expected in any fat transfer. Volume, site and plane are planned carefully, and the fact that the surviving proportion cannot be guaranteed is explained before any decision.

Recovery

Recovery after large-volume surgery is longer and more demanding: disciplined compression, early mobilisation to reduce thrombosis risk, lymphatic drainage, and swelling that persists for weeks to months. The full recovery schedule is explained before surgery, because being prepared for it is part of its success.

Limitations and risks

Risks include fluid loss and disturbance of fluid balance, fluid collections, venous thrombosis and its embolic complications, infection, fat necrosis, contour irregularity, asymmetry, altered sensation, and the need for staging or later revision — in addition to general surgical risks.
